    1. Download the form.
    2. If you are using Google Chrome, you need to close this browser before you open the document from your “Downloads” file.   This step is not required if you are using Internet Explorer or Firefox.
    3. Complete the required fields and click “Submit”. (The form should have a colored bar at the top as in the image below.)

    • You will receive an email from confirming that your request was received.
    • If you have any problems with the “Submit” function, simply do a “Save As”, and attach the completed document to an email addressed to:

    Please remember, students should try to create a schedule based upon the open courses available. Priority for requests will be given to students who cannot obtain schedules for required courses due to closed sections. Requests based on preference, including work schedules, are not always possible.
